Support Coordinator

Sacred Business Services (SBS) is an Indigenous owned business which supports mental health participants in all communities both remote and urban in the NT. We strive to make a difference in the lives of people with a mental illness by supporting participant-driven mental health recovery and assisting communities and organisations to actively improve their mental health and well-being.

 

Our programs aim to enhance the mental health and wellbeing of people living in all communities within the NT through a range of programs and activities including: 

·         NDIS funded support coordination, capacity building, living skills and social and recreational activities

·         Individual support to people experiencing mental illness

·         Tenancy support to people who are homeless or at risk of homelessness

·         A day program offering group activities and peer support

·         Mental health promotion to reduce stigma and raise community awareness

·         Support remote Aboriginal communities to build awareness and community capacity to implement suicide prevention strategies

·         Training in mental health first aid,

·         Advocacy for improved services at local, state and national levels.

 

Job description

 

Purpose of role

The Support Coordinator plays an integral role for NDIS participants in interpreting their NDIS plan, assisting them to establish the supports in their plan and develop their capacity over time to coordinate and manage their own supports.

 

Key Responsibilities

As a multi-disciplinary, professional team member, the Support Coordinator will enable effective recovery focused individual and group support through structured social, recreational, employment and educational activities.

 

1.     Assist NDIS participants to:

a.     Implement their plan and increase their capacity to manage/direct their own supports

b.     Have greater opportunities to explore and connect with their community and alternative support options

c.     Strengthen their informal support networks

d.     Maximise the value for money they receive from their supports

e.     Access and use the NDIS Participant Portal

2.     Effectively communicate and maintain relationships with the NDIA

3.     Action referrals in a timely manner

4.     Support the NDIS participant to:

a.     Assess a number of mainstream, community, informal and provider options

b.     Select preferred options or providers

c.     Negotiate services to be provided and their prices

d.     Develop service agreements and create service bookings with preferred providers

e.     Undertake initial client assessment to determine eligibility for engagement with the services offered by SBS.

f.      Determine the budget for each support type and advise any relevant plan manager of the breakdown of funds

g.     Liaise with any plan manager to establish the appropriate claim categories and attribute the correct amount of funds

h.     Link to mainstream or community services (eg, housing, education, transport, health)

i.      Strengthen and enhance participants capacity to coordinate supports, self-direct and manage supports and participate in the community, including providing participants with assistance to:

                                          i.    Resolve problems or issues that arise

                                         ii.    Understand their responsibilities under service agreements

                                        iii.    Change or end a service agreement

5.     Be the key contact for service issues, complaints, major changes and plan reviews.

6.     Assistant participants to prepare for their plan review by supporting them to:

a.     Assess whether they achieved their goals and got value for money for their plan

b.     Identify solutions to problems experienced in implementing the plan

c.     Consider new goals

7.     Maintain confidentiality of customers, their families/carers and the organisation

8.     Provide the NDIA with reports on specific goals, outcomes, and success indicators within the agreed reporting frequency

9.     Use assessment tools to support participants to develop, review and evaluate their recovery plans.

10.  Engage in supervision, training, planning and staff development activities.

11.  Actively contributing as a member of a team to the day to day functioning of the services, programs and facilities. 

12.  Actively contribute to creating and maintaining a safe environment for staff, participants and visitors. 

13.  Maintain a duty of care to staff, participants and visitors in accordance with SBS commitment to privacy and confidentiality requirements. 

14.  Actively promote the products/ services offered by SBS to relevant organisations and participants.

15.  Work collaboratively with other relevant services and organisations.

16.  Collect and store participant data and information according to SBS Policy and Procedures, Service Standards and funding body requirements.

SELECTION CRITERIA (required to demonstrate)

 

Qualifications

Certificate IV in one of Mental Health, Community Services, Allied Health Assistance, Youth Work or similar subject stream, or willing to work towards obtaining one, or equivalent level or work experience.

 Essential

 

1.     A demonstrated person-centred, recovery-focused, strength-based approach to participants’ care based on a sound understanding of mental health and illness

2.     The ability to work effectively and empathetically with individuals from diverse backgrounds, cultures and abilities. 

3.     An understanding of the issues that impact on people experiencing mental ill-health

4.     Knowledge of the National Disability Insurance Scheme (NDIS).

5.     Ability to understand the NDIS price guide and flexibility within budgets, the NDIS legislation and rules.

6.     Highly developed interpersonal communication skills including but not limited to:

a.     One-on-one group facilitation

b.     Relationship building with participants and agencies to encourage collaboration and partnership

c.     Advocacy and negotiation skills

d.     Conflict resolution skills

7.     Experience in basic case management

8.     Good time management, including independently establishing and managing work priorities and organizational skills

9.     High level of written and oral expression

10.  High developed computer and report writing skills

11.  Ability to work independently and within a team environment

12.  Current NT Driver’s License. 

 

Desirable

1.     First Aid Certificate

 

Conditions of Employment

The successful applicant will be required to undergo a National Police Check and possess an Ochre Card.
