These needs can include, but not be limited to, those relating to accommodation, activities, transitioning, translation, transportation and general household tasks.

Our Support Coordination services will help guide your through your NDIS journey and provide you support by:
Identifying the right local providers to suit your needs and taking care of the Service Agreement process
Coordinating with all providers and services to ensure that your needs are met
Finding suitable accommodating and respite options for you
Identifying suitable activities and/or employment options
Identifying and exploring solutions if you are in a position of risk
Helping you navigate key life-transitioning phases, such as moving
Monitoring your NDIS plan and reporting on your progress as required
